I can’t see/read anything on the 4x4 plate, is it a standard

You may be right, but the part number - 3031 is not visible on the plate (in
my experience, this does not happen very often).
Secondly, the quality of execution is very good, the photo shows that most likely
it is factory production.

I sent a request to Lego and I got this response:

I checked with my teams here across a few different departments, and it looks
like the reason why it was so tricky and why even through your great efforts
were you not able to find anything on the internet about it is because this isn't
an official LEGO® product at all.

It seems that this piece is some sort of DIY home-brew project someone through
together for some sort of custom application they were working on. What it was
intended for it anyone's guess. It looks like whoever built it combined one
of our 4x4 plate pieces and another one of our brick pieces with a 2x2 9V electrical
connector. We've never used a 2x2 9V electrical connector in any of our products.
