Why does this part have such a strange item number?

Other minifig heads have 4-digit numbers to identify the print. This one is a
single digit?

It is a leftover from the early days. There are numbers with "p", "px", "ps"
and "pb" I think it's something about the origin of the listing or something.
like p for printed and pb for printed, bricklink. There was a bit of chaos in
these days.
For legacy reasons this probably wont change now.
