I buyer recently bought a set from me and almost immediately after placing the
order I got a PayPal notification that I had been paid.
The problem is that I do not have instant checkout and send manual invoices.
So shipping was not included in their payment.
When I go to order received page the status still says pending but when I go
to add shipping to send an invoice it says it can't be changed because the
order has been paid.
Not sure how BrinkLink knows it has been paid if they went directly through PayPal
(I assume they did as the status didn't change and I am not sure how they
could go through BrickLink without an invoice.)
I have reached out to the buyer both through the messaging system, no response
after a few days, and today directly through email, so I will wait and see.
If I don't hear anything I will cancel and send the refund.

This is a foreign buyer with only 3 feedback, so relatively new but registered
almost a year ago.
Being an oversees buyer shipping is high where I don't want to eat that cost.
So a couple of questions come up.
Is this some kind of scam to see if I would just send the set? If they do pay
the additional shipping costs I will send it out but will still wonder if I should
be worried. Is this what you would do?

How can I add shipping costs and send a proper invoice to follow the proper channels
through BrickLink? Is this even necessary at this point?

And finally why does BrickLink think this order is paid? Am I missing something
on this one?

The buyer almost certainly marked it as paid - this can be done with certain
orders, I guess those that don't use an online payment method? My suggestion
would be to edit it and set the payment status to None/Not Yet Paid, give the
buyer a credit for the amount paid thus far, calculate shipping, and send the
invoice.

I found the setting Separate order payment status from order status, didn't
know this existed and did what you suggested.
Now I will have to wait and see.

Beware of this notification. Make sure it originates from PayPal and check your
account to see if you really have been paid. There have been reports recently
about a similar scam of fake e-mail notifications.
