In my opinion extras should only be present in a set inventory if they are found
in all or at least a significant number of sets. In this case it was probably
a packing error by Lego that led to an additional extra in the set that was used
by the submitter of the original inventory.
I have sometimes found unexpected extra parts in sets that were most likely the
result of mistakes in Lego's packing process. Especially extra sticker sheets,
but once or twice even parts that were not even present as regular parts in the
inventory of the set. In my opinion they should not be added as extras and I
would never submit them as such.
This is not the same as extras that vary between production runs of a set, for
example extra 1x1 plates that were added in later production runs of sets that
were originally released when Lego did not add those parts as extras. In that
case there are a significant number of sets that have these extra items and they
should be added to the inventory although I wouldn't mind a note for such
sets to indicate that those extras were not present in all production runs of
the set.

I haven't heard back from the submitter on this one, so I'm going to
chalk it up to a building error. Very easy to do - just leave one out and there
is an additional extra. The 5 times this plate is used in the instructions is
as follows:
p. 32 (back side of door)
p. 55 (lava lamp)
p. 88 (clear globe)
p. 90 (brown trophy)
p. 91 (telescope base)
