This set did come with one of these sheets of paper. An item for sale on eBay confirms this (eBay item number below). Interestingly, the box for this "set" appears to have been changed in 1993 to a box like this:

I am banned from adding photos anywhere on BrickLink, but this set with the older and newer versions of the box can be found on eBay by searching for item numbers 121845624286 and 151443912479. Would it be possible to add an inventory note regarding the box change, or would that be better as an additional note?
